Here's what we need from you in order to determine eligibility for our services:

Bid & Performance Bond Request Form, Here

Maintenance Bond Request Form, Here

Example of required info:

  1. Our Contractor's Questionnaire completed.
  2. Three years company FSs & tax returns
  3. One year pers FSs all owners& spouses
  4. Resumes of key people
  5. Our Reference letters: 3 Suppliers
  6. Our Reference letters: 3 Prior large contracts
  7. Our Reference letter: Bank
  8. Banking Resolution
  9. Interim FS on company
  10. Articles of Inc. or Operating Agreement
  11. Bond Request & supporting docs for project in question.
  12. Current Work In Process Schedule
  13. Copy of current Insurance Certificate (all lines.)
  14. Copy of current Workman's Compensation Insurance Certificate.
  15. Copy of Contractor's License or Assumed name Certificate (D.B.A.).
  16. Underwriting Fee of $750 for all new accounts.
